I affirm that I am duly authorized to complete the application and commit to this agreement on behalf of the business and authorize Fraser Form Rentals Ltd to make investigations of our credit standing as deemed to be necessary for this application. In consideration of Fraser Form Rentals Ltd extending credit to the customer for the purchase/rental of goods the customer agrees to the following terms and conditions for all orders placed by email, fax or phone.

Terms of sale are net 30 days. Payments on invoices issued to the customer are due within 30 days of invoice date. The customer attests to financial ability to pay Fraser Form Rentals Ltd invoices within the terms and conditions of sale. In the event the applicant's account is in arrears, any future purchases will be declined until the account is cleared, and the customer agrees to pay for all costs of collection or attempts thereof, including but not limited to attorney fees and costs, interest fees, and/or collection agency fees, for any amount in default. The customer acknowledges that all information provided in the Credit Application is true and correct.
